Uniform Complaint Form
The following procedures shall be used to address all complaints which allege that the district has violated federal or state laws or regulations governing educational programs. Please review the procedures provided on the left, and complete the form and submit it to the District Office.
Education Code 35186 creates a procedure for filing of complaints concerning deficiencies in instructional materials, teacher vacancy or misassignment, maintenance of facilities, and CAHSEE Prep. The following form contains elements required by Education Code 35186. You may return this form to your child’s school.

School Board Policy 5117 provides information on this process. For students transferring out of the district, please go to the school district's Welcome Center at 9800 Highway 53, Lower Lake, Ca to complete this form.
Home & Hospital Instruction Physician’s FormSchool Board Policy 6183 – A student with a temporary disability which makes school attendance impossible or inadvisable shall receive individual instruction in the student’s home or in a hospital or other residential health facility, excluding state hospitals. A copy of the policy, as well as the form, is included in this document.

Facility Use Board Policy

Facility Use Application
The Governing Board recognizes that district facilities and grounds are a community resource and authorizes their use by community groups for purposes provided for in the Civic Center Act when such use does not interfere with school activities.All school-related activities shall be given priority in the use of facilities and grounds under the Civic Center Act. Thereafter, the use shall be on a first-come, first-served basis.
